• Release of Information (releasing A Mindful You to communicate with PO that you are a client and the frequency of scheduled sessions. NOTHING in session will be released unless there is cause to break confidentiality)
• Client History (Non SO details)
• Client Insurance information (if any)
• Payment Authorization (for copays or cash pay sessions and assessment fees not covered by insurance)

Client is sent CASOMB packet

• Intake form
• Treatment plan
• Fee document
• Sex offender psycho social evaluation

(Needs to be printed and completed and brought to 1st session)
*Client can email completed CASOMB Intake document to office@amindfulyou.com for insertion into client file.

Patient is scheduled for clinic intake

* day 1 starts after completion of steps 2 & 3

First appointment intake and processing of all forms

Therapist completes assessments in session at 30 day mark

• LS/CMI (session 2 send completed doc to PO)
• Stable (session 2 send completed doc to PO)
• SOSE (Sexual Offender Social Evaluation) (session 3 send completed doc to PO)
• Treatment Plan (Sex offender specific NOT therapy notes) (session 4 send completed doc to PO)

Therapist to meet with PO monthly or more as requested by the PO

Sessions 4 through 52 are scheduled as weekly recurring appointments

*can be adjusted as needed for emergencies

After care therapy approval last 3-6 months with monthly therapy sessions

*(following the 52 weeks and getting therapist and PO agreements of downgrading care)
